outandabout on "[Plugin: WP Mail SMTP] test worked, but not with form"

I was able to get the settings adjusted so the test email worked and I'm sending emails via SMTP. I'm using the Artbees Ken wordpress template and am trying to incorporate one of their short code forms. The only real customization the form allows is where the information will be sent. When I fill out the form, I'm not receiving an email. Unfortunately, they are not giving me any help beyond 'look at the documentation'. Any idea if anything else has to be done to make this work?
